In The Binding of Isaac: Repentance, False PHD is a passive, Devil Room tier item that acts as a risk-versus-reward alternative to the standard PHD item. Instead of turning all pills into positive effects, it identifies all pills and converts positive effects into negative ones, but grants massive stat-scaling damage or Black Hearts as compensation. Immediate Bonuses: Spawns one random pill on the ground and grants +1 Black Heart upon pickup.
Pill Identification: Displays the name and effect of all pills before you consume them, allowing you to choose exactly which drawbacks you are willing to accept.
Stat Down Conversion: Consuming a “Stat Down” pill (e.g., Tears Down, Speed Down, Luck Down) permanently grants you +0.6 Damage. “Horse Pill” versions of these grant +1.2 Damage. This effect is retroactive; you get the damage for bad pills taken before buying the item.
Non-Stat Down Conversion: Consuming any non-stat negative or neutral pill (like Amnesia, Paralysis, Addicted!, or Bad Trip) drops a Black Heart on the floor. Strategy & When to Pick It Up
Sacrifice Excess Stats: It is an incredible item if your character is already near or at the maximum cap for stats like Speed or Tears. You can tank a minor stat down to infinitely scale your raw Damage.
Inconsequential Downsides: Some stat downs, such as Shot Speed or Range, have minimal impact on specific builds. Dropping your Luck stat below 0 does not penalize your room-clear rewards any further than having 0 Luck, making Luck Down pills essentially free damage.
Free Black Hearts: You can clear a floor entirely and pop an “Amnesia” or “??? ” pill right before leaving. The curse of the lost effect won’t hurt you because you are moving to the next floor anyway, but you still get a free Black Heart. Key Synergies
